Director of Martinsville-Henry County Health <br />Department, or his/her designee. <br /> <br />e. Executive Director of Piedmont Regional Mental <br />Health-Mental Retardation Services Board <br />(Chapter 10), or his/her designee. <br /> <br />f. Member of the City/County Nursing Home Pre- <br />admission Screening Team, to be designated by <br />the Director of the Martinsville-Henry County <br />Health Department. <br /> <br />2. The coordinating committee shall be authorized to appoint, <br />by mutual consent, such number of ex-officio members as <br />are determined to be qualified to assist and advise the <br />committee from time to time in the accomplishment of its <br />work. <br /> <br />3. The Henry County Social Services Department shall be <br />designated as the lead agency for the balance of calendar <br />year 1982 and for calendar year 1983. The City of Mar- <br />tinsville Social Services Department shall assume this <br />responsibility for calendar year 1984, and the responsi- <br />bility of lead agency shall thereafter rotate annually <br />between these two departments so long as the coordinating <br />committee shall function. <br /> <br />4. The role of the lead agency shall include: <br /> <br />a. Calling meetings of the coordinating committee. <br /> <br />,- <br /> <br />b. Acting as the liaison between the coordinating <br />committee, the City of Martinsville, Henry <br />County and the State Long-Term Care Council. <br /> <br />c. Assuring that a long-term care plan is developed <br />and submitted for approval to the City Council, <br />Board of Supervisors and State Long-Term Care <br />Council in a timely manner. <br /> <br />d. Assuring that data is gathered from members of <br />the coordinating committee and other service <br />providers and submitted to the State Long-Term <br />Care Council periodically. <br /> <br />C0uncil unanimously adopted the following ordinance, as introduced at Council's <br /> <br />September 14th meeting, which revises and wholly replaces the present Section 11-4 <br />
