Each Dependent Child <br />Elderly (62 years) <br />Disabled (other than elderly) <br />Low-Income (below 50% of median) <br /> <br />1 <br />2 <br />2 <br />2 <br /> <br />Structural Characteristics <br /> <br />Occupied <br />Moderate Repairs Required <br />Significant Repairs Required <br />Lacking Indoor Plumbing <br />Life Threatening Conditions <br /> <br />Following determination of income eligibility and Housing Rehab Board approval, the <br />Rehabilitation Coordinator will provide the Rehabilitation Specialist with a list of homes <br />for inspection, to include blower door tests. The Rehab Specialist will develop a work <br />write-up for the owner's signature and a cost estimate. The Rehab Specialist will also <br />prepare the bid packages. The Housing Rehabilitation Program Board will be provided <br />with a summary of each applicant and rehabilitation job for its approval prior to putting <br />the jobs out to bid. <br /> <br />Each applicant must establish clear title to the property. The owner will have to provide a <br />copy of the deed or mortgage. The owner can, however, provide proof of control of the <br />property if he or she has made payments of property taxes and insurance for the period of <br />ten years immediately preceding application submission. The Rehab Specialist will <br />determine and document whether an applicant has an emergency situation (e.g. roof leaks, <br />broken furnace, etc.) and Should be given priority. Properties found to be lacking <br />complete indoor plumbing will also receive priority ranking. <br /> <br />The Housing Rehabilitation Coordinator will accept applications at City Hall during <br />standard business hours, by appointment in the applicant's home, or by mail. The <br />Rehabilitation Coordinator will verify the information provided on the application after <br />determining that the application form is complete, including an executed "release of <br />information" authorization form that continues to be in effect for the duration of the <br />program. All applicants will be notified by letter as to their application status. Persons <br />whose applications are denied have 60 days from receipt of the letter to appeal the <br />decision and will be so notified in the letter. <br /> <br />Contractual agreements specifying the responsibilities of all parties must be executed <br />prior to the issuance of a Notice to Proceed. Documents to be executed include the deed <br />of trust, promissory note or loan agreement, and three-party construction contract. <br /> <br />12 <br /> <br /> <br />
