UNIFORM ASSESSMENT INSTRUMENT (UAl) - Used by human service <br />agencies to assess the need ~,nd eligibility for adult services. The 12 page UAl is <br />completed in its entirety for any In-Home services, Nursing Home preadmission <br />screenings, Adult Protective Services, and Adult Care Residence. <br /> · 154 Nursing Home preadmission screenings and Community Based Care <br /> · 27 Assisted Living facility <br /> · 29 Companion Services <br /> · 4 Adult Day Cares <br /> · 36 Assisted Uving reassessments <br /> <br />FAMILY SERVICES - Prevention and support services for families designed to <br />strengthen the family's ability to function more effectively and independently in <br />order to prevent family breakup or family violence. (Includes families with a <br />child(ten) likely to enter foster care; referral by the court for an adoptive home <br />study; interstate compact referrals for home study or supervision; referrals by the <br />court for a custody study or other services ordered by the Court and Child Care <br />services). <br /> · 429 ~ average number of families served <br /> <br />FAMILY PRESERVATION - Grants and other state funds provided to the <br />agency and community to preserve families, provide preventive activities to help <br />families nurture their children, and prevent removal of children from their homes. <br /> · Served 103 children and their families <br /> · ~75,280.80 disbursed en purchases of services for families and providers' <br /> <br />QUALITY DAY CARE INITIATIVE - Quality Initiative funds are made available <br />to agencies to develop, enhance and strengthen the quality of care delivered to <br />children. <br /> · ~;33,514.'14 awarded to day care providers in Martinsville and Henry Co. <br /> <br />INDEPENDENT LIVING GRANT -Services and activities for foster children to <br />promote self sufficiency. <br />30 youths 13 years and older were served by the Independent Grant. <br /> · ~;6,789.97 in financial assistance provided to 15 of these youths. · <br /> <br /> <br />
