CITY OF MARTINSVILLE <br />SCHEDULE OF ELEC~C SERVICE <br /> <br />Schedule: Outdoor Lighting (O.L.) <br /> <br />EFFECTIVE DATES: <br />City of Martinsville: 07/01/04 <br /> <br />Availability of Service <br /> <br />Available for outdoor lighting to individual customers <br /> <br />Monthly Rate <br /> <br />For each 9,500 lfigh pressure sodium (100 watt) <br />For each 50,000 lumen mercuxy high pressure <br /> sodium lamp (400 watt) <br /> <br />$9.65 <br />$8.77 <br />$14.81 <br /> <br />Minimum Charge <br /> <br />Monthly rates as specified above. <br /> <br />Special Terms and Conditions <br /> <br />Installation of outdoor lighting requires the customer to enter a special contract for a defined <br />period of time - 12 months for customers at the time of original installation. Subsequent tenants of <br />such premises may assume the remainder of any such contracts in a private arrangement with the <br />immediately preceding contractor, provided a copy of the written agreement is supplied to the <br />Utility Billing Division. If the terms of the original contract have been met the new tenant may <br />obtain the outdoor lighting by signing a contract and paying a $10.00 connection/transfer fee. <br /> <br />Once a customer has fulfilled the initial contractual obligation of 12 months, as the individual <br />circumstances dictate, service may be discontinued at the request of the customer for a period of <br />time and later reconnected for a fee of $10.00. This option is offered as a service to seasonal type <br />customers but is restricted until the original contract is met. <br /> <br />2. All lamps burn from one-half hour after sunset until one-half hour before sunrise, every night and <br /> all night, burning approximately 4,000 hours per year. <br /> <br />3. Credit on a monthly billing will be given to the customer in the event of outages in excess of three <br /> working days. <br /> <br />The City will provide, as a part of this service, the lamp, photo-electric relay control equipment, <br />luminaire and upsweep arm not over six (6) feet in length. This equipment will be mounted on an <br />existing pole or on a new one to be installed by the City which can be connected to existing <br />secondary circuits by one span of not over 150 feet. <br /> <br />A-10 <br /> <br /> <br />
