This decision was based on negotiated rates with Southern Health and specific performance <br />guarantees the company is prepared to meet within the benefit plan year. (See attachment). <br /> <br />Premiums are reflected in the table below, and are based on a $1500 in-network and $1500 out-of- <br />network deductible and a $10/$30/$55 drug plan with a $150 up-front deductible.
