GUIDELINES FOR EMERGENCY ASSISTANCE <br /> <br />Emergency Assistance may be provided after a thorough assessment has been <br />made by the social worker and it is determined that an emergency exists. <br /> <br />Emergency Assistance will be limited to the following: <br /> <br />A. Agency funds are limited to once per 12 - month period. <br /> Other resources have varying amounts and time limits. <br />B. Funds may be used to pay rent, (up to) $100.00 or electric (up to) $75.00 <br /> when a real emergency situation exists with cut-off notice and eviction notice. <br />C. Prescriptions for emergencies - RX $100.00 <br />D. Food <br />E. Other needs $50.00. Transportation assistance will be limited to a maximum <br /> of $10.00 in gas, only in valid emergency. <br /> *Fuel assistance (natural gas, propane gas, fuel oil, wood and kerosene) <br /> $100,00 <br /> <br /> Additional guidelines: <br />1. Limit the number of referrals to community resources (including the <br /> emergency fund) to three times in a 12-month period. <br />2. No further assistance for same need within a 12- month period. (Example: no <br /> help on electric bill twice in 12 months). <br />3. No assistance for fuel if customer has been or will be approved for the Energy <br /> Assistance Program, (Same for Electric heat.) Exception: Life threatening <br /> temperatures. No additional fuel if customer already has alternate heat <br /> source, such as kerosene heater. <br />41 Customer must be able to show how they can maintain in the future. <br />5. No assistance will be given the emergency was caused by mismanagement <br /> of funds. Refer customer to consumer Credit counseling. <br />6. If the need is greater than availableresourCes, customer is resp°nsible f°r <br /> paying the balance before the agency will authorize any payments. Customer <br /> should always be encouraged to pay a portion. <br /> 7. Assistance should get customer out of eviction or disconnect status. <br /> 8. Customers who give false information in order to receive assistance will be <br /> denied referrals. <br /> [ To receive help with rent, must have written eviction notice giving landlord's <br /> name, mailing address and phone number or a warrant or pay or-quit notice. <br /> If request is for first month's rent, must have a copy of lease or written <br /> statement of amount of rent, needed. Must be moving due to an emer,qency, <br /> not personal choice. <br /> <br /> <br />
