ARTICLE III - PARTICIPATION <br /> <br /> 3.1 Commencement of Participation. Each Employee whose customary employment, <br />excluding overtime work, is at least 40 hours per week, will be eligible to participate in the Plan on the <br />first day of employment. <br /> <br /> 3.2 Termination of Coverage. A Participant will cease to be a Participant as of the earlier <br />of (a) the date on which the Plan terminates or (b) the date on which he ceases to be an Employee <br />eligible to participate under Section 3.1. <br /> <br /> 3.3 Reinstatement of Former Participant. A former Participant who is reemployed by the <br />City of Martinsville and Martinsville City Schools will become a participant again if and when he meets <br />the eligibility requirements of Section 3.1 following the date of his reemployment by the Company; <br />provided, that if a former Participant is reemployment by the Company in the same Plan Year as a <br />prior termination of employment, then he may not make contributions for the Plan Year of <br />reemployment under Article IV which is inconsistent with his prior elections for the Plan Year. <br /> <br /> 3.4 Conditions of Participation. A Participant's receipt of benefits as a Participant in the <br />Plan shall be conditioned upon his agreement to: <br /> <br /> a. Furnish to the Administrator such information as may be reasonable required <br />from time to time on forms prescribed by the Administrator or otherwise; <br /> <br />b. Observe all Plan roles and regulations; <br /> <br /> c. Consent to inquiries by the Administrator with respect to any physician, <br />hospital, or other provider of medical care or other services involved in a claim under this plan; and <br /> <br /> d. Submit to the Administrator all reports, bills, and other information, which the <br />Administrator may reasonably require to process claims. <br /> <br /> <br />
