TICRC O[ <br />Amount of Award: <br />Award Date: <br /> <br /> Fund: <br /> <br />Enclosure <br /> <br />#6 <br /> <br />LETTER OF AGREEMENT AND <br />REPORTING REQUIREMENTS <br /> <br />By accepting economic development fimds from the Virginia Tobacco Indemnification and <br />Community Revitalization Commission, the grant or loan recipient ("recipient") hereby agrees to <br />the following conditions: <br /> <br />1. The recipient shall provide the Commission staff with an annual report due 13 months after <br /> receipt of the grant or loan. Such report shall contain the following information: <br /> <br />a) A summary of total project costs and total amounts expended to date <br />b) Source offimds expended <br />c) Details on how the funds from the Commission were used <br />d) Objectives accomplished during the year, based on original application for funds <br />e) Status of the project and estimated time and cost for completion <br />f) Report on any potential problems which will prevent timely completion of project <br />g) A copy °fthe most recent audited financial statements <br /> <br />This report, excluding financial statements, is to be no more than five (5) pages in length. A <br />reporting form is available from the Commission office. <br /> <br />2. The recipient hereby agrees to spend the Commission funds only in the manner outlined in <br /> the recipient's application to the Commission. <br /> <br />3.
